Synopsis: Perinatal outcomes were compared between 46 oocyte recipients of advanced maternal age and a control group of 49 advanced maternal age women who had conceived naturally. Multiple gestations occurred in 50% of the oocyte recipients. The multiple gestation group experienced an increased risk for placenta previa, PROM, preterm labor and delivery, glucose intolerance of pregnancy, pregnancy induced hypertension, and cesarean delivery. When single gestations were compared, the oocyte recipients had a lower rate of obstetric complications, except for the incidence of placenta previa (13.0% vs 2.1%). It was postulated that the higher risk for placenta previa is possibly due to the embryo transfer process, promoting a low implantation site in the endometrial cavity.
Comment: These results would appear to indicate that oocyte recipients have no additional risks solely because they are oocyte recipients. The issue of multiple gestations continue to be a major health concern in younger and older patients.
