Summary: The objective of this study was to determine whether circulating autoantibodies were associated with recurrent pregnancy loss. 74 nonpregnant women with a history of recurrent pregnancy loss and 75 controls of similar gravidity were tested for thyroglobulin and thyroid peroxidase antibodies and 3rd generation TSH . The blood samples were obtained 6 months after a pregnancy. Positive results for either one, or both, thyroid autoantibodies occurred in 22 (29.3%) of the women with a history of recurrent pregnancy loss and 28 (37%) of the control subjects. Mean TSH levels did not differ between the 2 groups. It was concluded that women with recurrent pregnancy loss are no more likely to have circulating thyroid autoantibodies than fertile control subjects. Testing for their presence is not clinically useful.
Comment: It is still useful to check a TSH level in all patients with ovarian dysfunction, although it is reassuring to note that autoantibodies do not interfere with pregnancy. It has been feared that patients with autoantibodies, although treated, have a higher incidence of pregnancy loss.
