New Drugs Get FDA Approval
Orlistat, a medication that functions non-systemically in the gastrointestinal tract to block fat absorbtion by approximately 30%, has received FDA approval for use in weight loss and weight maintenance when used in conjunction with a reduced calorie diet in obese patients with an initial BMI > 30 ( or > 27 in those with other risk factors such as hypertension, hypercholesterolemia or diabetes). There is also a therapeutic indication to reduce the risk of weight regain after prior weight loss. Due to Orlistats' mechanism of action, the most common adverse effects are oily stools and fecal urgency and associated gastrointestinal symptoms. Patients are advised to take daily supplements of fat soluble vitamins and beta carotene due to the reduced absorption of these vitamins. Orlistat is manufactured by Xenical, Hoffman-LaRoche Inc.
Rosiglitazone (Avandia, SmithKline Beecham) and Pioglitazone (Actos, Takeda America Research and Development Center Inc.) have gained FDA panel recommendation for approval for the treatment of type two diabetes. Both are thiazolidinediones. Rosiglitazone has been recommended as a monotherapy and in combination with metformin. Blood glucose levels were shown in studies (n=5,500) to have significantly decreased by 58 mg/dL with 4 mg and 76 mg/dL with 8 mg. There were no cases of drug associated jaundice or liver failure. Pioglitazone study group included 2,600 and also had no cases of drug associated jaundice or liver failure to report. The panel is still reviewing efficacy data on Pioglitazone.
