Frozen Section Proves to Be A Diagnostic Tool

Summary: Only histology can provide an absolute diagnosis as to the malignant or benign nature of ovarian cysts. The advantages of laparoscopic surgery over laparotomy are most notably: lower post operative pain; shorter hospital stay; shorter recovery time; and improved cosmesis. This study explored the role of performing a frozen section during the laparoscopic surgical management of organic ovarian cysts. 228 patients between the years of 1989 and 1996 underwent an adnexectomy for an ovarian mass. 26 (11.4%) of these patients were found to have signs suspect of malignancy restricted to the ovary. After extraction of excised tissues using an endoscopic bag during laparoscopic adnexectomy and followed by a frozen section, all 26 patients were found to have benign lesions as confirmed by histologic results. The authors feel that this laparoscopic strategy enabled the avoidance of a laparotomy suggesting theinclusion of frozen section in the laparoscopic surgical management of organic ovarian cysts. They also suggest that these encouraging results need to be confirmed by a larger sample size.
Comment: Laparoscopy has many many advantages over laparotomy. This is a logical and efficient application for the surgical management of organic ovarian cysts.
