ICSI in Immotile Sperm

Synopsis: Two cycles of ICSI resulted in transfer of normal appearing
cleaving using sperm from a 40 year-old male with 4 years of infertility,
normal semen except for total absence of sperm motility, and chronic pulmonary
infection.
Comment: This appears to be the first reported case of successful
treatment of this form of male infertility. It would be a more important
report, if a pregnancy had resulted, or if the patent had a more fully documented
medical and family history. Previously, it has not been known whether ICSI
would result in embryonic development in these patients. There is a spectrum
of inherited disorders affecting ciliated cells throughout the body that
been referred to as "sick cilia syndrome". By electron microscopy,
dynein a contractile protein in a sperm tail which is responsible for its
movement, is reduced, or missing. In the respiratory system, ciliated cells
clear mucus and secretions. An abnormality in these cells predisposes the
patient to chronic bronchial and sinus infections. In one instance, this
disorder is associated with situs inversus, a reverse in the location of
the heart to the right side of the body known as Kartagener's syndrome.
While these disorders are not common, one must be aware of the disorder
and its implications.
