Summary: Twenty obese patients with polycystic ovarian syndrome (PCOS) were followed in this prospective study. The objective was to determine the clinical, hormonal and biochemical effects of metformin after 4-6 months of therapy. Metformin therapy consisted of treatment with 0.5 g of metformin 3 times daily for 4-6 months. Evaluation parameters included: clinical symptoms, menstrual pattern, hirsutism, serum concentrations of sex hormone binding globulin (SHBG),gonado- tropins, and lipids. Regular cycles were attained by 11or 68% of the women. Hirsutism, body mass index, and blood pressure were unchanged. The mean testosterone levels decreased after 2 months but returned to their original levels by 4-6 months. Free testosterone decreased significantly. The levels of other sex steroids or lipids measured did not demonstrate any change. The authors feel these results support that metformin is easily tolerated by most patients and maybe clinically useful in obese patients with PCOS and menstrual disturbances. Testosterone levels suggest the effect of metformin may be transitory and hirsutism was unaffected in this particular study.
Comment: The earliest studies reported Œcross the boardı improvements in a number of endocrine and metabolic parameters. Later studies, including our own on-going trial, have shown that obese tend to lose weight and this has a clearly positive metabolic and overall health benefit. Pregnancies are occurring in a variety of patients that previously have been quite resistant to conventional therapy. Some commentators have suggested that the positive effects of metformin seem to be related to weight loss alone, but there seems to be more to it than that. Certainly, we will see more on this
