Ovarian Cancer: Should We Screen?

Summary: Studies have shown that ovarian cancer can be detected before symptoms are present by the measurement of Ca125 levels, ultrasound, and color flow Doppler. This randomized trial assessed multimodal screening with sequential Ca 125 antigen and ultrasonography. Women, aged 45 years and older,were randomized into a screening or control group. Screening participants were offered 3 annual screens for Ca 125 and if the level was over 30 U/mL pelvic ultrasonography was performed. Of 468 women in the screened group, 29 women had abnormal an ultrasound and 6 had ovarian cancer. The positive predictive value was 20.7%. 10 more women developed cancer in the next 7 years. In the non screened group 20 women over the span of seven years developed ovarian cancer. Median survival for the screen group was 72.9 months and in the control group, 41.8 months. Death from ovarian cancer did not differ significantly in the two groups. The author's suggest a larger randomized trial to see if screening affects mortality.
Comment: We continue to look for a way to diagnose ovarian cancer early, the statistical analysis for screening remains iffy and the population cost high. Still, routine ultrasound may have a role.
