Summary: Two hundred twenty-eight women receiving routine prenatal care during the first trimester and who had a living singleton fetus were examined for microbiologic flora of the vagina in an effort to discover a link between first-trimester diagnosis of bacterial vaginosis and cessation of pregnancy at <20 weeks' gestation. Researchers found that the presence of bacterial vaginosis at the first prenatal visit was strongly associated with subsequent early pregnancy loss. After multivariate analysis, bacterial vaginosis, Mycoplasma hominis, and Ureaplasma urealyticum, but not other microorganisms, remained linked to a greater risk of miscarriage. It was concluded that bacterial vaginosis and mycoplasmas may play contributing roles in spontaneous abortion and early pregnancy loss.
Comment: To culture for these organisms is very expensive and not very reliable. It is possibly better to treat, but to whom and when should we give antibiotics.
