Summary: Researchers studied the effect the site of application has on 17-b estradiol bioavailability. They conducted an open-label, randomized, crossover study of a once-weekly transdermal estradiol patch (Climara). Patches delivering 0.1 mg per day of estradiol were placed on the subjects' buttock or abdomen. Researchers obtained serial plasma samples over 7 days and for the immediate 24 hours after the patch was removed. Plasma estradiol concentrations were sustained at premenopausal levels over the week in most subjects. After application on the buttock, average peak plasma concentration was 125.1% and average relative bioavailability was 117.2% of that from the abdomen site. Researchers concluded that the buttocks are an acceptable site for the application of this once-a-week 17-b estradiol transdermal delivery system. The fact that the absorption was considerably greater for the buttock than for the abdomen indicates that this application site may provide an advantage in women who experience menopausal symptoms at the end of the week.
