Summary: Postmenopausal women who have a vertebral fracture are at an
increased risk for experiencing an additional fracture within the following year,
is what researchers concluded after a 3 year osteoporosis treatment trial. It has
been documented that vertebral fractures increase lifetime risk of future
fractures. However, the risk of additional vertebral fractures in the period
immediately following the initial fracture had not been previously evaluated.
Researchers examined data from a group of 2725 postmenopausal women with a mean
age of 74 years who had been randomized to a placebo group and for whom vertebral
fracture status was known at entry. In the first year of the study, the
cumulative incidence of new vertebral fractures was 6.6%. When compared with
women who had no prevalent vertebral fractures at baseline, those with greater than or equal to 1
fracture at entry were 5 times as likely to sustain an additional fracture during
the first year of the study. A total of 381 women experienced an incident
vertebral fracture during the 3-year study period. Of these women, the incidence
of a second vertebral fracture in the following year was 19.2%. Risk was also
increase in the presence of prevalent vertebral fractures. Researchers believe
that their findings suggest a need for urgency in identifying and treating
patients who have vertebral fractures. Their data also suggests that osteoporosis
may be a quickly progressing disease once a fracture occurs.
Comment: The key is prevention.
