Hot Flashes Before Menopause

Summary:A hot flash is a neuroendocrine event related to estrogen withdrawal. Because of its association with ovarian failure, the occurrence of hot flashes in women of reproductive age has gone largely unrecognized. This observational, prospective/retrospective cohort study reports the occurrence of hot flashes in women of reproductive age. Patients with confirmed PMS(157) were compared with women identified as having ³chronic menstrual cycle-related symptoms² (27). Women without these symptoms and without PMS were used as controls (58). Classification of PMS and identification of menstrual cycle symptoms was accomplished using the Prospective Record of the Impact and Severity of Menstrual Symptomology Calender (PRISM). 83.4% of the PMS group and 81.5 % of the ³chronic menstrual cycle-related symptoms² group reported one episode of chills and sweats (hot flash). This compared with 43.1 of the control group women reporting one episode of a hot flash. The authors concluded that women with cyclic and chronic menstrual cycle symptoms appear to experience episodes of chills and sweats similar to menopausal hot flashes.
Comment:Surprised? Vasomotor instability should be and is a sign of alteration in ovarian function. Flushes/ flashes near the time of menses is a frequent sign of beginning the menopausal transition. Many of these women may benefit from a cycle directed short course of estrogen therapy.
